Leads care transitions, population management, discharge planning, follow-up care, staff supervision, budgets, and regulatory compliance.

Summary


GENERAL SUMMARY:

The Care Management Manager provides leadership and supervision of the key functions, roles, systems and processes related to care transitions and population management into and out of the medical center. Assures the delivery of quality, patient centered care in a manner reflective of the organizations’ mission, vision, values and beliefs. Possess managerial and clinical skills appropriate to the scope of responsibility. Manages the cost centers’ fiscal, material and human resources emphasizing staff retention strategies. Participates in department, division and organizational wide initiatives focusing on regulatory compliance and performance improvement.

PRINCIPAL JOB FUNCTIONS:

*Commits to the mission, vision, beliefs and consistently demonstrates our core values.

*Develops and initiates service/program activities for the organizational coordination of care transitions including the areas of care management, navigation, population management, discharge planning and follow-up post-acute care that is based upon evidence-based best practice and efficacy.

*Develops, implements and analyzes adherence to organizational and service specific standards of care and policies and procedures.

*Empowers staff to participate in committees/councils and to operationalize the vision, beliefs and patient centered principles; mentors staff in quality performance efforts and fosters a culture based on continuous improvement.

*Engages in management, clinical and organizational meetings as appropriate. .

*Assists with the department’s capital, operational and human resource budget based on strategic service priorities, best practices and organizational standards; analyzes and resolves variances.

*Recruits and manages staff to demonstrate behaviors consistent with Bryan beliefs and Bryan Core Values; acts as chief retention officer for areas of responsibility.

*Conducts timely staff performance appraisals assuring staff competence while emphasizing staff retention strategies through coaching and individual development.

*Timely, objectively and constructively coaches staff as appropriate when practice and/or safety issues are observed or reported; ensures appropriate corrective actions are completed for staff when there is a failure to meet job performance expectations.

*Ensures that meaningful leader/employee rounding is completed according to span of control expectations.

*Resolves clinical concerns by patients, families, and physicians; promotes medical staff relationships.

*Ensures ongoing compliance with accrediting and regulatory standards.

EDUCATION AND EXPERIENCE:

Master’s degree in Social Work required. Licensure as a Licensed Clinical Social Worker (LCSW) and Licensed Mental Health Practitioner (LMHP) by the State of Nebraska required. Minimum of three (3) years recent clinical experience required. Prior care transitions management experience preferred. Prior supervisory or management experience preferred. Case Management certification preferred.

OR

Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. Bachelor's degree required, master's degree preferred. Prior care transitions management experience preferred. Prior supervisory or management experience preferred. Case Management certification preferred.

OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required.
