Case Manager
Clinically based case management supporting patient care and resource utilization on the Ortho unit, working with case management and multidisciplinary teams. Requires a Missouri RN, a BSN, 2-3 years' case management or related healthcare experience, and computer skills; ACM preferred within 1 year.
SUMMARY: Under the direction of the Supervisor Transitiono f Care, provides clinically based case management to support delivery of effective and efficient patient care. This position partners with the Case Management and multidisciplinary teams to identify appropriate utilization of resources and to ensure reimbursement.
This position supports the Ortho unit.
EXPERIENCE: Minimum 2-3 years' experience in Case Management or related healthcare field.
SPECIAL SKILLS: Excellent communication, interpersonal, organizational and presentation skills.
OTHER: Computer skills.
LICENSE/CERT: Required: RN (Registered Nurse) State of Missouri Preferred: ACM (American Case Management) within 1 year of hire
EDUCATION: Required: Bachelors - Nursing
