Discharge Facilitator II
Processes patient referrals for discharge planning with bedside case management. Part-time Monday-Friday, 6-hour days, benefits eligible; requires 1 year in the medical field, computer skills, and preferred related bachelor's degree.
SUMMARY: Under the direction of the Supervisor of Care Continuum and collaboration with the bedside Case Management team, processes referrals for patients services to meet their discharge planning needs, assures a cost and quality effective discharge, and always promotes patients' rights and maximum benefits from health care services.
This position is part time, working Monday - Friday, 6 hr days 8am-2pm or 10am-4pm. Position works 60 hours in a 2 week pay period and is benefits eligible.
EXPERIENCE: One year in the medical field (i.e. hospital, home health or nursing home). Prefer on year discharge planning or processing referrals. Insurance coverage exposure is a plus.
SPECIAL SKILLS: Excellent communication, interpersonal, and organizational skills.
OTHER: Computer skills including Microsoft Office and electronic health records software.
